I wore it 3 times, but the OP is still in good condition. The only small stain is around the label (see picture), which is not visible when wearing. I will also clean that before selling.
Measurement is as below:
Shoulder: 40cm
Bust: 98cm
Waist: 88-100cm
Length: 105cm
I have put two pictures of myself wearing it as a showing. My own measurement is shoulder 42cm, Bust 96cm, Waist 78cm, and height 178cm. This OP suits me very well. It's good for tall Lolita girl like me:)

Hi, I just made a quote from USPS website, it says that from my zipcode to yours, the cost of Priority Mail 2-Day™ Small Flat Rate Box is $7.15, so I suppose the actual cost to be close:)

The cheapest way to ship to Canada is through air mail for items below 4lbs. But USPS does not supply shipping cost estimate online, since the priority mail fee is around $24, I will suppose the shipping cost via airmail to be around $10-$15.

USPS Priority Mail 2-Days flat rate box cost is estimated to be $7.15. So I suppose the actual cost should be around it..
