Bodyline's Twirling Heart Polka Dot JSK, released in 2011. It's perfect for old school or retro inspired coords, and has cute details like heart shaped lace and a polka dot bow holding the back of the shoulder straps together.

There are some manufacturer's flaws such as the mesh on the lace not being fully trimmed off, and the waist bow in the stock photo was not included.

It can fit a US size M or L quite comfortably.
