Could this fit a 54 inch waist or be altered without deluding the stitching

Hello Fae, the largest the waist goes is 74cm which is about 29 inches. I've never altered this dress but if you're good with sewing and stitching it may be possible. The waist has little plastic boning to help keep its position so I am not sure if that would hinder or help you. Whatever you decide I hope this helped :)
